Heart Health Monitoring Goes Mainstream
Smartwatches have evolved into genuine health monitoring devices. In 2026, several models offer FDA-cleared ECG readings, blood pressure estimation, and irregular rhythm notifications validated in clinical studies.
How We Tested
We compared five smartwatches against medical-grade equipment: 12-lead ECG, cuff-based blood pressure monitor, and chest strap heart rate monitor. Testing included resting, exercise, and overnight scenarios.
Results
Best Overall: Apple Watch Ultra 3
ECG app shows 98.5% agreement with reference 12-lead for detecting atrial fibrillation. Heart rate accuracy during exercise was within 2 BPM of the chest strap across all activities.
Best Android Option: Samsung Galaxy Watch 7
BioActive sensor delivers reliable ECG and blood pressure monitoring (requires monthly cuff calibration). Sleep apnea detection is the first FDA-cleared consumer screening feature.
Best Battery Life: Garmin Venu 4
Seven days of battery with continuous heart rate monitoring. ECG is FDA-cleared and accurate. Body Battery energy monitoring provides useful recovery insights.
Most Innovative: Withings ScanWatch 2 Nova
Traditional watch face with medical-grade sensors. Vascular age assessment measures pulse wave velocity — a marker of arterial stiffness. 30-day battery life.
Feature Comparison
- ECG: Apple and Samsung lead in accuracy.
- Blood pressure: Samsung requires monthly cuff calibration.
- Irregular rhythm: All five watches offer this; Apple has strongest clinical validation.
Important Caveats
No smartwatch replaces a medical evaluation. These are screening tools that flag potential issues requiring professional follow-up.
